STUDENT IDENTIFICATION

 

 

A state identification number shall be assigned each student upon enrollment, in accordance with Louisiana Board of Elementary and Secondary Education requirements.

 

EMERGENCY INFORMATION

 

At the beginning of each school year, students shall be required to complete emergency information forms which shall contain such information as:

 

  1. Parent/guardian’s work phone numbers

  2. Home phone numbers

  3. Phone numbers where parents/guardians can be reached in case of emergencies

  4. The name, address, and phone number of the student’s physician

  5. The names of authorized persons to check-out and/or pick up students in the absence of the parent/guardian. 

  6. Other information as may be requested on the form

 

In addition, standardized school health forms shall be completed by the parents/legal guardian for each student.  These health/medical forms provide pertinent health, medical and emergency contact information in times of need.

 

IDENTIFICATION CARDS

 

The DeSoto Parish School Board, noting that school safety and security are of extreme importance for all schools, establishes this policy concerning student identification.  If the principal chooses to implement the identification card program, then the following guidelines shall be followed:

 

  1. Students who are enrolled in the school must be identified by the school computer management system showing the student's name, address, parent/guardian's name(s), date of birth, and dates of entry or departure from the school.

  2. Students in Grades 6-12 must have a current student identification card showing students name, school, bar-coded student identification number, and the student's picture. Student identification cards may be used for school lunches, checking out library books, and checking in or out of school.  The cards must not be defaced or altered in any way.  Cards altered or defaced must be replaced.

  3. The first student identification cards shall be provided each year by the DeSoto Parish School Board at no cost to the students.  Lost or misplaced identification cards may be replaced at a cost of $3.00 per card.  "Temporary" cards will be provided to students who do not have an identification card (first temporary card is no charge but next cards will cost $1.00 each).

  4. All visitors must wear a "Visitor" card while on any campus during a school day showing that they are a visitor to the campus.  Exceptions will be students who have an identification card from a visiting school or when large groups come onto the school campus.

  5. Students (Grades 6-12) who fail to follow proper use of identification cards can be disciplined accordingly by the school administrators.
